There are two factors that can affect the duration and interval of your menses at this point.
The first is that, you are at an age where natural menopause may already be setting in, and these irregularities in menses may be part of this natural process.
Second, this could indeed be a side effect of the tamoxifen, since this really disrupts the normal hormonal rhythm. Some women would even have 2-3months in between menses while on tamoxifen.
Regardless of the menstrual interval, it is important that the thickness of the endometrium (the inner lining of the uterus) is monitored every 3-6 months while on these types of medications.
There is a cut-off thickness of the endometrium (about 10mm), when the tamoxifen should be stopped, or when there would be an indication for a currettage.
I agree that you would need a transvaginal ultrasound at this point to evaluate the status of the endometrium.
