OnTamoxifen for DCIS. Periods are two days long but vary from 18-45 days between. Have pre-DCIS fibroids. Are 18 days between a problem?

Asked by cabo (Female; 46; Low grade DCIS, 4mm removed in biopsy last year. Clean lumpectomy, obviously large margins. No rads recommended, on tamoxifen. Have fibroids pre-DCIS, being watched with vaginal ultrasounds every six months; Relevant drugs:tamoxifen, synthroid ) on Sun 13, Jul 2008 07:40am

My cycles come every 28-45 days on tamoxifen. They are about two days long, sometimes spotting starts for a couple of days prior. This month it came again 18 days from the first spotting of last month. The onc's nurse said to see what happens next month. I have been on tamoxifen 6 months. I had a vag/us 5 months ago as a baseline. I'm freaked out about uterine cancer. I realize it's a small risk. Should I have another vag/us or just see what happens?? Thank you for this service.

Answer by Dr Paul S, MD  on Mon 14, Jul 2008 09:55pm:

Hi there. There are two factors that can affect the duration and interval of your menses at this point. The first is that, you are at an age where natural menopause may already be setting in, and these irregularities in menses may be part of this natural process. Second, this could indeed be a side effect of the tamoxifen, since this really disrupts the normal hormonal rhythm. Some women would even have 2-3months in between menses while on tamoxifen. Regardless of the menstrual interval, it is important that the thickness of the endometrium (the inner lining of the uterus) is monitored every 3-6 months while on these types of medications. There is a cut-off thickness of the endometrium (about 10mm), when the tamoxifen should be stopped, or when there would be an indication for a currettage. I agree that you would need a transvaginal ultrasound at this point to evaluate the status of the endometrium. Regards and God bless.
