Bladder cancer with no metastasis - what is the cause of death?


    
Asked by zeus (Male; 71; My father was diagnosed with CIS bladder cancer approximately 3 years ago. He underwent BCG treatment (unsuccessful). By that time it had progressed to T2N0M0. He then had a chemo/radiation regimen, which was successful. However, the cancer recurred about 1 year later and was called T4N0M0. It recurred this time on the outside of the bladder wall, not the inside. He underwent 2 GC chemo treatments, but had to be hospitalized about 1 month ago due to unmanageable pain. He died several days later. ) on Wed 07, May 2008 12:03pm

I know you can only make general statements without all of the specific medical details, but I’m wondering what, more specifically, caused his death (besides the general conclusion “cancer”). Previously, I believed that cancer killed by eventually spreading throughout the body until the function of vital organs was destroyed. I now know this is not always the case, since his was only a localized 2cm tumor. There was no distant metastasis. Although he hardly had any quality of life at the end, his urinary function was accomplished by a catheter, and he had some pain management with opioids. What happened? Did the tumor deplete his body of essential chemicals or substances, did it disrupt other biological processes, did it overwhelm his immune system, etc.? Was it just a general overwhelming assault on all of the body’s functions and defenses? Any explanations you can offer would be helpful to my understanding.

Hi. I'm sorry to hear what happened. It will be indeed very hard to ascertain the cause of death from this venue but here are some points. Those who die of cancer usually have wide spread disease, or have tumors in vital organs such as the lungs, brain, or liver that compromises their function. Though he did not have this, a localized tumor to the bladder could still have caused damage by compressing other nearby structures such as major blood vessels. This can result to bleeding or thrombosis that can be fatal. The pain may also cause excessive sympathetic activity resulting in increased heart rate, increased blood pressure and overall, can cause the cardiac function and activity to deteriorate. This can be possible especially in the elderly, whose cardiac functions can be compromised in the first place. Some side effects of chemotherapy like neutropenia can cause a patient to be susceptible to overwhelming infections which can be hard to treat. Cancer patients are also susceptible to thrombotic events (formation of clots in the blood vessels) which can lead to a condition called "pulmonary embolism" (clots clogging the blood vessels in the lungs), which is usually sudden and may appear unexplained. Here are just some pointers and I believe it is better to discuss this more with the doctors who attended to him. My condolences to you and your family. Regards and God bless.
