Complex Hyperplasia-Hysterectomy-full or partial to prevent ovarian cancer?


    
Asked by Unregistered (Female; 49; hypertension, overweight,skin cancer (basal cell) two years ago, family history of diabetes, heart disease, colon cancer (father had all of these.) non smoker; Relevant drugs:lotensin 5mg. over the counter vitamins ) on Fri 15, Aug 2008 08:34pm

I am a 49 yr. old perimenopausal female diagnosed with endometrial hyperplasia (complex). My doctor is recommending a hysterectomy. He also suggested that he could remove my ovaries at the same time to prevent ovarian cancer. Does endometrial hyperplasia put me at an increased risk for ovarian cancer? I am concerned about removing the ovaries if I don't need to. I do not have a family history of ovarian cancer, but my father did have colon cancer-does this put me at a higher risk for ovarian cancer? Is it wise to be placed in surgically induced menopause for the purpose of avoiding ovarian cancer?. Could this put me at a greater risk of breast cancer, osteoporosis and heart disease? Am I better off leaving the ovaries in and letting nature take its course in terms of the start of menopause?

Hi, Endometrial hyperplasia is a condition of excessive proliferation of the cells of the endometrium, or inner lining of the uterus. Most cases of endometrial hyperplasia result from high levels of estrogens, combined with insufficient levels of the progesterone-like hormones which ordinarily counteract estrogen's proliferative effects on this tissue. Ovarian cancer has been known to be affected by a lot of reasons, some of which are related to the hormone and pregnancy status of the person. It may also be related to other cancers such as gastrointestinal cancers in the family as you have indicated. It is unclear to me from this post that what are the reasons other than family history of GI cancer which may prompt your surgeon to include an oopherectomy. You may get another consultation or ask the same doctor why he thinks you should get this done, in spite of havinf any other symptoms. Endometrial hyperplasia is not directly linked, other than the fact that the ovarian cancer may be related to hormonal changes, which are seen in many cases including endometrial hyperplasia. The second part of your question is also true and I would agree that menopause would put you at a greater risk for these conditions. However, the degree of benefit and risk may need to be analyzed in your case. let me know your thoughts/inputs/queries if any. Best regards
