Asked by user (Male; 52; high blood pressure, type II diabetes, otherwise fairly healthy; Relevant drugs:blood pressure, sugar meds ) on Wed 10, Sep 2008 02:50pm

Husband had total thyroidectomy due to stage III papillary thyroid cancer a year ago, 2 cancerous nodules returned below the thyroid bed, he had surgery today to remove nodules...I spoke with his Doc (general surgeon) right after the surgery and he said the surgery went well and he got what he was looking for and when I asked him if he got it all...he said something that troubled and shocked me...he said "I'm sure there will be more"...I didn't even know what to say to that I was in shock...Can someone explain what that means?...Does it mean it will recur local as those did, or to distant sites, such as lungs, bone...etc...??? I have left him a message on his voicemail, but probably won't talk to him till tomorrow...I am very concerned.

Answer by Dr Paul S, MD  on Wed 10, Sep 2008 07:54pm:

Hi. It is in the very nature of cancer cells to have the tendency to grow back and indeed spread to other organs. the fact that the cancer has recurred once, makes it all possible of it returning again whether locally or at distant sites. The second surgery only removes what are visible to the naked eye, and the theory is that there can still be microscopic cancer cells remaining (also called 'micrometastases'). To lessen the chance of the cancer returning or spreading, I suggest you discuss with his doctors regarding additional treatment options such as radioiodine therapy (if he did not receive this before or if his previous dose is not yet maxed out). Regards.
