I am wondering if I should be getting a biospy and what are the chances that I have cancer?


    
Asked by Unregistered (Female; 35; YES; Past medical- I've had a lot for a 35 yr old white female weigh about 120- 125 lbs - hear is the short list, cervical cancer @ 30 yrs old, brain aneurysm clipped @ 32 yrs old , infertility 30-32yrs old, had high blood pressure for years which in the last couple of months went back to normal, migraines, asthma, GERD, 2 small right breast lumps ( came back not cancer); Relevant drugs:no meds regularlly- migraine, asthma and GERD meds are all as needed ) on Thu 26, Aug 2010 01:57pm

I am wondering if I should be getting a biospy and what are the chances that I have cancer? Here are my symptoms... Swollen lymph nodes in groin ( least 5 that I can feel) , armpits ( under both armpits) and neck (above my collar bone on left side- this one is perfectly round). They all range in size from 1/4 to 1/2 inch each. I have three under my right armpit and they seem to be connected. I first noticed one swollen lymph node in my groin last January but now they seem to be everywhere. They don't seem to grow very fast. None have disappeared. Other issues are- hair loss, fatigue, diaherra, nausea, headaches Past medical- I've had a lot for a 35 yr old white female weigh about 120- 125 lbs - hear is the short list, cervical cancer @ 30 yrs old, brain aneurysm clipped @ 32 yrs old , infertility 30-32yrs old, had high blood pressure for years which in the last couple of months went back to normal, migraines,asthma, GERD, 2 small right breast lumps ( came back not cancer) PCP did CBC, liver function & sexually transmitted disease and everything came back normal for the exception of a low red blood count. Please help...

Hi! Many thanks for your question on the forum and a warm welcome on board. I am so sorry to hear about all that you have gone through. Swollen lymph nodes across multiple areas or generalized lymphadenopathy can be due to viral infections such as mono or EBV or cytomegalovirus or due to bacterial infections like TB. HIV or AIDS, leukemia, lymphomas and other cancers can also be a cause. This could also be due to inflammatory conditions such as rheumatoid arthritis and Crohn’s (likely as you have diarrhea and anemia). With your medical history possibility of cancer will always remain and hence a more aggressive approach towards diagnosis should be adopted. Since CBC is normal, a peripheral blood smear should be done to look for abnormal or precursor cells. Montoux test for TB should be done and blood tests to detect infectious mononucleosis. RA factor should be tested and so also ANA or antinuclear antibodies. Stool should be tested for parasites, worms and infection. Before biopsy, a non invasive test like CT scan of a lymphnode from each site should be done. If nothing comes up, then biopsy should be done. Do discuss this with your PCP and get a referral for an oncologist to get a more clear picture. Hope this helps. Please let me know if there is any thing else and do keep me posted. Take care!

Comment by Dr Ajit Singh on Fri 27, Aug 2010 03:33am: Hello need advice please, welcome to ask doctor for free.com. Dr. Kokil Mathur has left no stone unturned and fairly justified your post. Considering your history of cervical cancer, the most likely possible medical condition that needs to be ruled out first is Urethral Cancer and lymphoma. But as per the present symptoms what you have mentioned {hair loss, fatigue, diarrhea, nausea and headache}, it could also be Tubercular Inguinal Lymphadenopathy {Tuberculosis} or Hyperthyroidism. Get a thyroid profile and FNAC {fine needle aspiration cytology} done to rule out these conditions. But gold standard is excisional biopsy of the lymph node. However non invasive methods have to be applied first as also said by Dr. Mathur. Meanwhile start on with anti diarrheal, simple analgesics, anti emetics and vitamin E supplements for time being. Kindly have your family doctors opinion to proceed further and have an Oncologist consultation at the earliest. Hope this additional information helps you.
