Organs that has received radiation in the past, may easily be injured and the immune defenses of these organs may not be that strong. Hence, these can be prone to infections and this may explain the bleeding that your father recently experienced.
Another possible cause is the presence of stones in the urinary tract which can likewise cause this bleeding.
