MALT lymphoma

Asked by grp (Female; 48; good health; Relevant drugs:lipitor and zetia ) on Tue 11, Nov 2008 08:48am
Priced at $29.00

I had been diagnosed with h. pylori infection in September. I was treated with triple therapy -prevacid, biaxin, and amoxicillin- for two weeks. I finished therapy October 4. I was still having some symptoms, so I had an upper endoscopy done yesterday. The nurse said exam was normal, but doctor took some biopsies anyway. I know she told me she would take a stool antigen test. But I am worried about MALT lymphoma. Would that or any other abnormality have shown up during the endoscopy? My mother had NHL of the gastrointestinal tract-doctors classified her as a high grade B cell lymphoma and said her cancer was rare. She did die from it very quickly. Are there any symptoms with MALT lymphoma? Thank you for your help. I have had CT scans with contrast and barium, and a colonoscopy, and all was normal. My blood work came back normal, also.

Answer by Dr Dennis S, MD  on Wed 12, Nov 2008 06:18pm:

Hi. Marginal zone B-cell lymphoma of MALT (Mucosa Associated Lymphoid Tissue) is an extranodal type of lymphoma. The stomach is the most frequent site of occurrence, hence the term gastric MALT lymphoma. Other less frequent sites include the lung, orbit, thyroid, and salivary gland. H. pylori infection is associated with the development of gastric MALT lymphoma, as well as gastric adenocarcinoma. There is a five- to sixfold increase in risk of gastric malignancy, including adenocarcinoma and lymphoma. Most patients with gastric MALT lymphoma complain of vague abdominal pain similar to that of peptic ulcer disease. Rarely do they complain of systemic symptoms typical of patients with lymphoma. Endoscopy is the best way to diagnose this disease entity. Biopsy of suspicious sites can be done. Treatment of H. pylori positive gastric MALT is antibiotic and follow-up endoscopy after 3 to 6 months. Those with partial response should receive a second course of antibiotics. Patients with H. pylori negative gastric MALT should receive radiotherapy initially because they will not respond to antibiotics intake. Hope this helps. Good luck and take care.
