More info regarding previous question about colon cancer - Invasive grade 2 adenocarcinoma of the colon with invasion of pericolonic adipose tissue and venous involvement of the mesenteric margin of resection

Asked by tcbryant (Male ) on Thu 20, Mar 2008 12:56am

Male, 58 years old, good physical condition, hypertension under control with meds, no heart problems; Invasive grade 2 adenocarcinoma of the colon with invasion of pericolonic adipose tissue and venous involvement of the mesenteric margin of resection. Metastatic adenocarcinoma invoiving 1 to 8 pericolonic lymph. sigmoidectomy length 15cm; tumor site sigmoid colon; tumor configuaration ulcerating; tumor size 5 cm; histologic type adenocarcinoma; histologic grade low grade (well to moderately differentiated). Gross description: Adhesions involving the surface of the mesentery that slightly distort the segment of bowel. Slicing these reveals a serosal surface of the colon to be generally smooth with an area of whitish-tan discoloration and retraction located appx 4 cm from the nearest stapled end. The segment is opened revealing a circumferential ulcerated sessile mass lying 4 cm from the nearest margin. It measures appx 5 cm in curcumference, 5 cm in length and .7 cm in maximum thickness. Within the mesentery beneath the colon & seperated from the colonic wall by appx 1 cm of grossly unremarkable appearing adipose tissue is a moss of what apppears to be a necrotic tumor possibly involving lymph nodes. It measures 1.5 cm in diameter. Can you provide me with a more clear picture with this new information?

Answer by Dr Heinrik M, MD  on Fri 21, Mar 2008 01:40am:

Hi, In terms of 5 year recurrence rates, the estimate would be 84% without any treatment after surgery. Since he received chemotherapy, the rate is improved to 49%. In terms of 5 year survival, the estimate without any treatment after surgery would be about 21%. Since he received chemotherapy the survival improves to 45%. It is advisable to keep a schedule for surveillance. As was mentioned in the previous query, metastatic disease is not always incurable. Stay positive.
