Recommendation for Chemotherapy or not (Breast Cancer)

left breast, lumpectomy performed, clear margins, 1.7cm invasive ductal with "lobular characteristics", sentinel node biopsy negative (1 node removed), bone and CT scans negative, stage I, grade II, oncotype RS=26, ER=9.3, PR=3.3, HER2=8.9 right breast, MRI identified microcalcifications and fibrious densities covering area approx 1.4cm, MRI guided biopsy with 12 samples determined to be benign. chemotherapy decision: yes or no?

Hi. From the National Comprehensive Cancer Network (NCCN) guidelines on breast cancer treatment, any cancer that is more than 1 cm should derive benefit from adjuvant chemotherapy. Your recurrence score of 26 is also rather high or in the gray zone. For this reason, the present recommendation is still to give chemotherapy as your tumor may have an increased risk of recurring or spreading. There is an ongoing clinical trial evaluating the giving of chemotherapy for tumors in the gray zone but until the results of such become available, chemotherapy is still recommended. Regards.
