The 12 spots on the lungs may indeed be from the bowel cancer, and if can be proven as such, then the patient is said to have metastatic disease. The test which can determine if the spots are malignant is called the FDG-PET Scan with or without CT scan. If the spots are malignant, they would "light up" on PET scan. If still equivocal, a biopsy may have to be performed.
CEA may not always be elevated in bowel cancers or in their recurrence. some bowel cancers do not produce this substance and for some, it is not a reliable indicator of disease progression.
Another possibility, especially in heavy smokers, is the possibility of another primary cancer in the lungs (primary lung cancer, and not metastatic cancer from the bowels). This may explain the normal CEA levels.
I suggest you discuss these possibilities and options with your partner's doctors, and I hope everything will turn out fine.
