I am 48 and female. I have had 2 transvaginal ultrasounds and three Ct scans since May because I was having gastrointestinal issues. One of the ultrasounds and 2 Ct scans picked up a fibroid tumor 2.5cm. I had a thorough pelvic exam and doctor felt nothing abnormal. I talked to nurse about usg results and she said I have adenomyosis. What is that? She said it was not cancerous and not to worry. But I am. My periods are regular but are getting further apart-last month I waited 40 days, and I am usually 26-32 days. They last 3-4 days in duration and I do not have heavy bleeding or bleeding or spotting between periods. No painful intercourse. Even the ulstrasound was not painful at all. I am not anemic either. Can I live with this or will it turn to cancer? The first day or so of period I pass small clots and I have menstrual pain, but if I take Motrin I am OK. I don't understand why some tests picked it up and others did not. I do not want a hysterectomy. I will be 49 in 4 months. Should I get second opinion or get MRI? Thank you for your help

Hi there. First, it is best to verify the diagnosis, whether it is a fibroid tumor (leiomyomas) or that of adenomyosis. These are two very distinct clinical entities. Leiomyomas are tumors of the smooth muscle of the uterus and is quite common among women in your age group. This is a benign tumor, and is usually diagnosed through the appearance on imaging studies as smooth, well demarcated, or well encapsulated nodules or mass. If there are reasons to point to something more serious (such as heavy bleeding, irregular tumor borders, weight loss, etc), then your doctor may request a currettage or similar sampling procedures, or proceed outright to hysterectomy. Adenomyosis on the other hand, is the extension of the endometrium (the glandular layer) towards the myometrium (the muscle layer). This is indeed not cancerous and does not turn into one. Those without any symptoms like the ones you mentioned may be followed up regularly and intervention only done if you develop these symptoms. Unfortunately, even an MRI or any other test would not be 100% sure in detecting adenomyosis, and the sure way to diagnose this condition is through pathologic examination once the uterus is removed surgically. However, your doctor or gynecologist may do a currettage if she thinks there is reason to suspect a more serious condition. I suggest you talk to the doctor him/herself to verify what you really have, and if your doctors are confident that this is nothing to get worried about, then I would suggest to stop all the worries.
