my father has multiple myeloma..iga at diagnosis is 5050..after one round of velcade it went to 5700. after first infusion of second round was hospitalized from continued lumbar lesion pain. while in hospital, found iga to be 4700..also, creatanine went to from 2.3 to 1.3...doc though wants to discontinue velcade and go to revlimid..feels we did not see a significant reduction in iga.. my question is, should we give up on velcade after a little over one round? have heard we need at least two rounds to see a response? also, how treatable is this disease.. will thalomid or revlimid help further reduce dad's cancer. we are very afraid and concerned for him..thanks

Hello,

Reassessment for response to velcade takes into account various parameters, like symptoms (bone pain), serum Ig, calcium, decrease in bone marrow plasma cells, decrease in anemia, improved renal function etc. Usually, a fair trial to a chemotherapy drug or regimen is given before declaring that it is not working in a particular patient and changing to next-line chemotherapy.

In your fathers case, I am sure that the treating oncologist has taken a comprehensive view, and you must discuss with him/her in detail.

There are several promising drugs for myeloma, and outcomes have significantly improved over the last couple of decades.
