I am 78 years old. Two weeks ago I had my upper right lobe removed due to a tumor (6.6 cm). I have never smoked. It was cancer; poorly differentiated adenosquamous carcinoma, nuclear grade 3, with focal metastatic adenocarcinoma (each 2.0mm) in two of the four lymph nodes they removed. Tumor was 'Extensive invasion of bronchial wall' but 'resection margin was free of tumor'. This is my 4th bout of cancer. 1986 breast cancer, 2004 other breast cancer, 2005 Edometrial Adenocarcinoma resulting in Hysterectomy due to cancer. By the Pathology department reviewing slides from my 2004, 2005, 2008 cancers, it ws determined that my current 2008 cancer was related to my Uterus cancer. I had a PET/CT and Bone Scan next. Bone Scan revealed cancer in my T9 area. "Fairly intense localized activity'. It suggests active metastatic disease. Thus now my cancer is graded a 4 Question is:according to statistics, what is my life expectancy? I do not want to know a 5 yr survival rate rather an median or average month life expectancy. I am average weight, decent physical condition although red blood cells remain low. I slso had two knee replacemtns since 2000. Thanks.

Answer by Dr Paul S, MD  on Sat 30, Aug 2008 06:19pm:

Hi. Due to the multiplicity of your previous cancer primary sites, it is indeed very hard to estimate a median time of survival. However, basing on your doctor's conclusion of a metastatic ovarian cancer, your median survival will parallel that of stage IV ovarian cancer cases. Without further treatment, stage IV ovarian cancer patients have a median survival of about 12 months. With chemotherapy, this median survival can further increase or even double at more than 24 months (some studies even quote up to 36 months). Metastatic sites that are completely resected (such as your lung lesions) can add even more time to your survival, and there have been documented long term survivors even with a stage IV disease. Here is also a site I found that discusses chemotherapy regimens and median survival times: http://clinicalevidence.bmj.com/ceweb/conditions/woh/0816/0816_T4.jsp I suggest you discuss with your doctors regarding additional chemotherapy to increase the chance and length of survival. Regards and God bless.

Clarification by sunny on Sun 31, Aug 2008 01:33pm:
Thanks for the answer. Both my surgeon and Oncologist believe I am not a canidate for Chemotherapy due to
age, and all the surgeries and Chemo/radiation I have had past few years, but we will be discussing it more.
I understand that my lung may have been completed resected, but alwyas thought once cancer gets into the bones
it meant you had a short life left.
And, with the cancer in my T9 back bone, I figured I had a pretty short life expectancy. Can one live for a
year with no treatment with cancer in the bones?

Comment by sunny on Sun 31, Aug 2008 01:38pm: To clarify: I know one can live for a year under this situation, but I assumed my life expecancy under my dituation with the cancer in the bones was more 3 to 6 months.
Comment by Dr Paul S, MD on Mon 01, Sep 2008 11:20pm: Hi. On the contrary, there have been reports that cancers with pure bone metastases can have longer survival times (even more than 5 years), as long as there are no other tumors in other places. Medications for the bones called the 'bisphosphonates' like Zometa or Aredia can be given to limit bony destruction by cancer.
