It seems you have been through a lot.
The lobular variety of breast cancer seems to have a predisposition for involving both breasts. The fact that you had a prophylactic mastectomy and still had the nodes involved raises suspicion if you have any genetic predisposition for breast cancer. While there are tests to ascertain this, the problem is you have had 2 mastectomies already, and hormonal therapy. Hence, whether or not you do have the genetic predisposition, you have undergone the treatment that would be offered to you (which is the mastectomies and the hormone therapy.
The current testing with CA-125 now raises the possibility of ovarian cancer. Risks for ovarian cancer can also imply a hereditary pattern that may or may not be related to the breast. Testing at this point will raise the intervention by means of yet another operation (which would mean removing both your ovaries). But like the breast removal, in which surrounding tissue may still develop cancer – then removing the ovaries cannot also make the risk of recurrence zero. Medications to limit the recurrence are available, but the hormones used here run contrary to the breast cancer treatment (meaning, drugs to prevent the ovarian cancer may promote breast cancer recurrence) and hence, won't be suitable for you.
For the general population in which a new ovarian cancer is found, only around 10% have a genetic predisposition. But in your case, it is likely much higher than this so if the testing is available, then perhaps you should consider it.
