testicular cancer

Asked by kristen5751 (Male; 30; STem cell transplant in 2006 for NHL Testicular cancer chemotherapy April - July 2009 Treatment for NHL in 2003 and then relapsed in 2005 and that is the reason for transplant; Relevant drugs:none ) on Sun 26, Jul 2009 03:55pm

My husband is 30 years old and 3 years ago he underwent an auto stem cell transplant. He did well and recovered wella fterwards. His counts returned to normal. His wbc never fully got normal, but everything else did. In April of this year, he was diagnosed with testicualr cancer. HE had 4 rounds of chemo and he finished July 2. We have been going back for cbc checks and his counts have dropped some since he finished treatment. Especially his rbc. Last week it was 6.7 and this week was 7.9. He did receive a procrit shot last week. Will his counts recover? IF so, how long could this take. He wants to feel better!! HE is an active guy, and being tired is not fun for him. I am worried about him. The doctor said the cure rate for his testicualr cancer is excellent, but I am now concerned about his blood counts. His platelets last week were 18 and this week were 88. Does that mean everything should be on the rise? IS there a chance his rbc may only recover to 12 and if so, would he be ok?

Answer by Dr. Monica, MD  on Mon 27, Jul 2009 08:21am:

Hi, Chemotherapy drugs can cause myelosuppression i.e. decrease the production of blood cells in the bone marrow which consequently leads to low blood cell counts. Once the chemotherapy is completely the bone marrow function recovers gradually. The rate of recovery depends on the reserve of blood forming cells in the bone marrow which in turn depends on the dose and duration of chemotherapy given. Other therapies given for cancer such as radiation therapy can also cause temporary or sometimes permanent bone marrow suppression. His counts are already improving as is shown by his blood counts this week. This indicates that he should in all probability recover completely in a few days or weeks. Meanwhile hematinic supplements,a nutritionally balanced diet and a positive mental attitute will help to hasten the recovery. All the best!

Comment by kristen5751 on Tue 28, Jul 2009 09:50am: Other than blood counts is there any other way to see if he is getting better. We only see the doctor once a week. Even with having treatment 6 years ago due to NHl, then a transplant 3 years ago due to reocurrence, and now testicular cancer, can he still have good bone marrow??
Comment by Dr. Monica, MD on Wed 29, Jul 2009 07:50am: Yes, it is possible that his bone marrow reserve is still good since his counts are recovering fast. He needs to follow up regularly with his oncologist. Serum levels of germ cell tumor markers such as Alpha feto protien, Beta HCG, Lactate dehydrogenase need to monitored periodically if the cancer was a non seminomatous germ cell tumor . An increase in the levels of these markers can indicate a relapse of the testicular tumor especially if these markers were elevated initiallly at the time of diagnosis. All the best!
